Jacob F. Ort, age 23, formerly of Farmingville, NY and Kentucky, died Saturday, July 4, 2015.
He is survived by his loving step-father Andrew and mother Brenda Nailboff, sister: Monica Chumley and brother: Justin Plate. Memorial Visitation will be held Friday, July 10 from 2 PM - 4 PM at Rocky Point Funeral Home, 603 Route 25A, Rocky Point, NY. for the little time I have know Jacob aka jake he was a nice respectful kid that came to my shop to get his hair cut I’m sure he will be missed my. Condolences got out to his family and all his friends for there lost RIP Jake
